INDIVIDUAL COACHING POLICIES & PROCEDURES - One Session
By clicking on the "I AGREE" link below, client hereby engages the services of Barb Elgin (DBA ‘BE A Success Enterprises’ and 'Coach Sappho') as their “Coach” for the purpose of coaching and supporting with respect to:
- Reaching personal objectives
- Improving interpersonal and/or personal relationship skills
You - Ideal Participant Profile
- Has a positive attitude toward personal and/or professional growth
- Is willing to take action to achieve her/his goals
Coaching Calls - You and I will meet on the telephone for a total of one, 30 minute session. Coach calls client if client in continental US. Outside of US, client calls coach. If you will be at any other number other than the one you provide to me, you will provide that number to me ahead of time. Client will pay $25 via credit card in advance of the session at coach's website.
Changes - If the call needs to be rescheduled by either you or me, 24 hours notice is requested. There will be no refund or credit for cancellations made less than 24 hours prior to a scheduled session. However, in the event of an emergency, or if our usual session falls on a holiday, each of us will make every attempt to reschedule appointments at a mutually convenient time. If you are not available when I call, I will try to reach you to find out what happened. You agree to provide me with specific instructions as to where/how you want to be contacted, what information is okay to leave as a message and an emergency contact.
Confidentiality - Our discussions will be considered confidential and I shall use reasonable care in maintaining confidentiality. I make no express or implied warranties or guarantees and will not be held personally responsible for any inadvertent violation of confidentiality unless it is proven to be willful with foreseeable direct, material harm to you. You understand that wireless communication, email, fax, efax and other online methods (instant messenger, chat rooms and discussion boards) may not be secure and that archives of email communication may be subject to electronic interception or may be kept by third parties (such as employer-based voice mail and email systems, so keep this in mind if you decide to communicate with me from work) and be subject to court orders. The client understands that there may be exceptions to coach-client confidentiality that may include responding to court orders. And in the event of discovery of child or elder abuse or the client's intent to cause harm to herself/himself or others, the coach will take all reasonable and appropriate action which may include breaching confidentiality.
Communication - This agreement constitutes the complete agreement between coach and client and no other representations have been made regarding outcome. As the client, you understand that these sessions are consultative in nature and that any actions taken and decisions made are solely your responsibility. As your coach, I make no guarantees or warrantees, expressed or implied, about any results to be achieved. The coaching process is designed to support an individual in making and following through on choices that further his/her personal and professional development. While there are high expectations of professional and personal growth as a result of coaching, there are no guarantees. The results are directly related to changes someone is willing and capable of making in his/her attitudes, behaviors, habits, and approaches. We understand there are no guaranteed results from this program and we are solely responsible for making each of our own decisions, taking any actions, and creating our own outcomes. We realize that there may be unintended effects of taking this program and we take full responsibility for those effects and hold coach harmless.
The coach will challenge, encourage, and support the Client as well as make suggestions. The options are for the Client / Participant to accept, decline, or make a counter offer. Please do not agree to an action you are unwilling or unable to take. If the coach asks you to do something that does not feel right, please bring it to her attention so it can be discussed. The Client / Participant is fully responsible for any decisions made and actions taken.
I understand these coaching services do not include:
- Expert advice particular to my business, field, or industry
- Financial or legal advice
- Therapy
- Training
- Psychological counseling
I understand these coaching services do not include the promise of:
- Attainment of specific objectives or outcomes
From time to time coach may provide client with information regarding other providers of services or goods. All referrals made by the coach are subject to independent evaluation by the client and no warranty or representation regarding their services is made by the coach.
In the event I feel a need for professional counseling or therapy or if the coach recommends same, I understand that it is my responsibility to seek the services of a licensed professional.
I understand there are no guaranteed results from this program and I am solely responsible for making my own decisions, taking any actions, and creating my own outcomes.
The coach has not been represented to me as having any particular expertise, experience, education, or background other than what has been provided to me in writing.

I certify that I am 18 years of age or older and am able to sign a legally-binding contract. I agree that I am responsible for notifying Barb Elgin, in writing, of any changes to the above information.
